II. General Observation
The patient is a 15-year-old female, stands about 5’1”, has healthy body build, brown complexion, and has straight, black hair. She was wearing pink shirt, yellow pants, red body bag, and blue pony tail. She has two moles on her face.
When queried why she came at NCMH, she just claimed gusto kong magpa-doktor”. She was with her mother. When the patient asked if she thinks she’s mentally ill, she claimed “oo”.
She’s the eldest among the brood of three siblings whose are Len-Len and Nerissa. Her father, 34 years old, is a farmer at Nueva Ecija, and her mother, 36 years, is a housewife.
The patient is currently not studying, she stop schooling after what happened to her. She was a rape victim, as what her mom claimed “kinukuha nya na daw nuon yung mga sinamapay sa labas ng bahay, wala siyang kasama, bigla daw syang tinutuan ng baril nung kapit bahay naming mag-aama at dinala sa bahay nila, doon, inuntog daw sya sa pader hanggang sa mawalan na ng malay. Nuong pag kauwi nya sa bahay namin, ibang iba na sya, sobrang tahimik at parang takot na takot.” “nalaman ko na ni-rape sya ng mag-aamang De Guzman ”The patient, as claimed by her mother, has been very disturbed after that unpleasant incident. “lagi nya sinasabi na patayin ko nalang daw sya”, her mother also claimed.

In assessing the verbal and performance test, the patient obtained impaired to her alertness to her environment, numerical reasoning, and ability to see relationship between things and ideas. Her visual memory, her ability to analyze abstract reasoning and visual motor speed are also impaired.
Bender Visual Motor Gestalt Test
As an outcome to the given test, patient is emotionally disturbed so that maintaining adequate interpersonal relationship is difficult for her. She has a depressive condition due to her unexpected sexual experience
Draw a Person Test
The patient manifested withdrawal tendencies, she actually wished to die because of excessive fear she felt brought about by her bad experienced, her unexpected sexual experienced.
Sach’s Sentence Completion Test
As a result to SSCT, revealed, the patient’s feels completely incompetent hopeless and feels she is mentally defective.
V. General summary
Subject current intellectual ability is on the Moderate Mental Retardation range and Innate Potential is Moderate Mental Retardation also. Verbal and performance test are impaired.
Patient is emotionally disturbed, interpersonal relationship is difficult for her, she has a depressive conditions, and withdrawal tendencies, she actually wished to die because of excessive fear she felt brought to her bad experienced, her unexpected sexual experienced.

my very FIRST PATIENT....

II. General Observation
The client is an adult female, stands approximately 4’9” with a thin body build and has a dark complexion. The subject was wearing a white and blue stripe t-shirt and a maong pants. She was wearing sandals which she kept on removing while answering the given exams. The client has a dark, short, and wavy hair, she has lots of scars and open wounds in her arms and feet.
The client was with her aunt and since she doesn’t have the ability to talk, the interviewer chose to talk to her aunt. As the patient’s aunt asked if what would be the reason why they came at NCMH, she just claimed, “eh kasi requirement ito ng fiscal para sa kanyang kaso.” The patient’s aunt refused when asked if the patient, she thinks is suffering from mental illness and claimed that the only problem is that, the patient talk to herself very often and murmuring which can’t understand by anyone. The patient’s aunt stated that the patient’s fear became excessive after what happened to her. “Iniwan ko sya noon sa bahay na mag-isa dahil may bibihilhin ako sa labas, napadaan ako sa kapitbahay at napasarap ang kwentuhan namin, pag balik ko naabutan ko sa bahay namin yung lalaki na dali-daling nag bihis at tumakbo, yung pamangkin ko naman ay naginginig sa sobrang takot,” the patient’s aunt claimed as queried if what actually happened to the patient.
About the patient’s family background, her aunt stated that the patient is living with her already, she became her guardian since the client’s mother died. According to her, the patient has two siblings, when queried, she just claimed “tatlo silang magkakapatid, sya ang pangalawa, yung bunso namatay at yung panganay naman ay nakakulong.” Her oldest brother was accused of being thief and was in jail since last year. Their father left them since then.
While the patient is having her psychological tests, she was easily get distracted by any sounds or noise she will hear, her eyes kept on moving, very observant and very suspicious.

As an outcome to the given examination, the patient indicates mental retardation which firstly reflects her inability to complete a task due to her limited understanding and capacity to express herself. The subject also shows hesitancy, self doubt and nervousness as a result of her unexpected sexual experience.
Draw a Person Test
As a result to the exam given, the patient shows low self esteem and timidity and has depressive tendencies by the fact that she has traumatic sexual experience. The patient also indicates a feeling of inferiority, with poor manual skill due to her unresponsiveness and incapability to talk.
V. General summary
The patient indicates mental retardation, inability to complete a task due to her incapability to talk which also lessen good communication with others. She also shows low self esteem, timid, shy and has a depressive tendencies and designates traumatic psychosis that caused by her unexpected sexual experience.

-it was really a memorable one for me, first reason is because, she is the very first patient i handled.. second, admitted ako na medyo nahirapan ako sakanya kasi sobrang hirap nya makausap kasi lagi syang walang reaction and di talaga sya nagsasalita... but i really enjoyed the experience.. very challenging.. worthwhile and very appropriate and suitable sa kung anu talaga ang gusto ko!! :)
